Knee issue

I've been feeling some mild irritation on the front of my knees. (both knees)

Felt like it's either just behind the kneecap, or slightly above the knee cap. Kind of vague as to exactly where.

Usually while/after riding. So it's clearly cycling triggered.

I recall noticing it coming on since the last year, or the year before that. At first, it would come on during the ride and disappeared after a day of rest. Now, it's more noticeable, for it's there a bit more often, since I bike commute almost daily.

That happened to coincide with the start of my Citibike membership. I'm wondering if my daily grinding of a heavy bike with less than perfect seat positioning/geometry has anything to do with it?

Anyone had experienced anything similar to help shed some light?

Trying to figure out what to do or avoid so it doesn't progress to anything worse.

Saddle fore-aft position.

Those darn rent-a-bikes are great in a pinch but someone like you, all dialed in and serious, is likely to demand too much from them. After a while they demand too much in return.

See a Doc

I had this pain and it was Patellofemoral Syndrome. After correct diagnosis physical therapy cured it completely.

But until you get a correct diagnosis, which may involve films, you or anyone else is just guessing. I know you don't care for docs you've seen, but a good knee specialist likely can tell you exactly what it is and what to do about it. And a good doc won't recommend surgery unless you need it.

I'm with Dan. And yes, bad position will exacerbate it.

My patellar tendions give me shit unless its around 70 or I warm up really easy.

Thanks Dan!

Patellofemoral Syndrome describes my symptom to a T!

I'm actually in the middle of a PT series for IT band syndrome, another long standing issue I put up with for a while but finally decided to deal with. I'll ask the therapist if she can suggest some exercise for me to try. Going back the doc at some point. If it doesn't improve, I'll ask him to take a closer look.

How is your knee on you own bike VS citybikes? Although once aggravated it may be too late which bike caused it to matter.

I already got a MRI of the knee. That was for my complain on the side of the knee. There's nothing pathological wrong about my (left) knee.

So I was sent to PT to deal with the tight IT band. It's tight on both side, but much worse on one than the other. It's starting to show some result.

My impression of orthopedics are they don't spend much time in diagnosis. :( If the condition doesn't look like operable, sent them to PT and wash hands! Next patient please.

I'll add my own experience with Citibikes. I ride around 5,000 miles a year spilt between a road, gravel, and occasional mountain bike with no pain or discomfort. But one short ride on a Citibike makes my knees and hip hurt (not bad but enough to worry and stretch later). I think it's the combination of weight, saddle position, and lack of pedal licking to keep my feet in one place.

Just to clarify a bit. There's not much pain, not yet.

But I definitely felt some inflammation and some tenderness. The level of inflammation is about the same as a year ago. So it hasn't progress too much just yet. The main concern is mostly because it doesn't seem to go away even when I'm not riding. (the irritation "calms" down when I don't ride for a few days, but just never quite completely clears up)
