Water bottles vs. dishwashers

Obviously not, but a little closer to it than rinsing with hot tap water. This was my point in case it was missed.

I am not comfortable rinsing with bleach, although considering how much pool water I have downed in my life [talk about bacteria]...

But I will stick with my habits. Elaine had me brain washed, hand washed, and a bunch of other ones too.

keep the tops open / off

I've found that keeping the bottles open to the air helps keep the growth to a minimum.

I've heard people say to keep empty camelback bladders in the freezer to prevent the growth of the nasties. I've haven't tried that yet.

Wife and I use the Clean Bottle which Stan mentioned.
They are coming out with an Apple bottle, well...sort of.
It is a square bottle designed by a couple of Apple engineers.
Their video introducing the product is clearly a guy who is trying to sound like a Steve Jobs product intro.

I agree, tops off, anything else being "the problem" imo.

I noticed long ago that most modern bottle tops have the inside protrusion for the valve assembly, which hooks nicely and somewhat securely on the edge of the bottle opening. This keeps the cap with the bottle while at the same time allowing a plentiful evaporation window.
